It's a skin condition called "Vitiligo", a genetically caused malfunction of melanine production. 1–3 % of all people are affected with it. It's, of course, especially noticeable if you have a darker skin tone.

First class technique and subject, but the mother seems a bit too plump and her skin condition is disfiguring.

About the "plumpiness" of the woman - well, I'd say that this lies in the eye of the beholder, especially given that she's pregnant ...
and about her skin condition - it's called "Vitiligo", and yes, that might be seen by some as disfiguring, but 1-3% of all people just are affected with it.
In the imaginary time/culture, in which this scene is supposed to be, people with vitiligo are seen not as disfigured, but as special - because of their rarity.

Thanks for getting back to me on this matter.  I did not realize she was pregnant.  In Africa vitiligo is considered disfiguring for blacks.
